Non-UCLA Participant: $135.00 if postmarked by 9/7/2019
GI Technician Participant: $120.00 if postmarked by 9/7/2019
UCLA & Cedars Employee: $120.00 if postmarked by 9/7/2019

Cancellation Policy
Written requests for cancellation must be received by 9/7/2019. All fees except $25 will be issued one month after the seminar.
